To help you effectively manage these conditions, we’ve established a chronic care management program. When you need extra support to manage a chronic care condition, we enroll you in this program. This includes nursing staff who regularly checks in with you about your condition and progress, plus remote patient monitoring that reduces incidents of hospitalization and emergency room visits.
